Ran across a nice case of arteriolar hyalinization in a hypertensive/diabetic patient. This is a 60-ish man with longstanding hypertension and diabetes mellitus II found dead at home. He had a history of increasing weakness and weight loss over the past few months. At autopsy, he had the usual hypertension/diabetes changes, though his kidney was a bit different than what I usually see. His subcapsular scarring wasn’t all that bad, and his arcuate and radial arteries were only middling changed. However, he had pretty impressive hyalinization of his arterioles. The decedent’s vitreous creatinine was 4.4, estimated GFR 12. Here’s some pics. Unfortunately, he was a bit autolyzed, but it’s the thought that counts:
