{"id":522,"date":"2020-10-01T09:22:35","date_gmt":"2020-10-01T13:22:35","guid":{"rendered":"http:\/\/wordpress.forensicpath.us\/?p=522"},"modified":"2023-11-24T21:36:48","modified_gmt":"2023-11-25T02:36:48","slug":"pathology-cases-cardiac-amyloid","status":"publish","type":"post","link":"https:\/\/wordpress.forensicpath.us\/index.php\/2020\/10\/01\/pathology-cases-cardiac-amyloid\/","title":{"rendered":"Pathology cases &#8212; cardiac amyloid"},"content":{"rendered":"<p>This is a case of a young middle-aged woman with a history of drug use found dead at home with drug paraphenalia nearby.\u00a0 At autopsy, she had high levels of both methamphetamine and fentanyl.\u00a0 However, on anatomic evaluation, she had an enlarged and dilated heart (620 g).\u00a0 That isn&#8217;t all that uncommon in my jurisdiction, since many intravenous drug users have endocarditis\/valvular disease with resulting functional impairment.\u00a0 However, in this case, the valves were a little thickened, but there were no vegetations.\u00a0 Instead, the heart was rock hard to the touch.\u00a0 It felt as if it had already been fixed in formalin.\u00a0 On histologic examination, there was a lot of interstitial fibrosis, some of which had that amorphous hyaline look you see in amyloid.\u00a0 So, I got a Congo red, and sure enough, it had the characteristic &#8220;green apple&#8221; birefringence.<\/p>\n<p>Here&#8217;s an H&amp;E section of the interventricular septum at the septal base, with a portion of the atrioventricular node (click on the image to see the 7Kx4K version):<\/p>\n<figure id=\"attachment_526\" aria-describedby=\"caption-attachment-526\" style=\"width: 688px\" class=\"wp-caption alignnone\"><a href=\"https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-scaled.jpg\"><img fetchpriority=\"high\" decoding=\"async\" class=\"size-large wp-image-526\" src=\"https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-1024x557.jpg\" alt=\"H&amp;E atrioventricular nodal region\" width=\"688\" height=\"374\" srcset=\"https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-1024x557.jpg 1024w, https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-300x163.jpg 300w, https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-768x418.jpg 768w, https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-1536x835.jpg 1536w, https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-2048x1114.jpg 2048w, https:\/\/wordpress.forensicpath.us\/wp-content\/uploads\/2020\/10\/cardiac_amyloidosis_HE-1600x870.jpg 1600w\" sizes=\"(max-width: 688px) 100vw, 688px\" \/><\/a><figcaption id=\"caption-attachment-526\" class=\"wp-caption-text\">H&amp;E section of septal base near atrioventricular node<\/figcaption><\/figure>\n<p>As an aside, I recognize the nonuniformity of the background here, but it&#8217;s hard to correct for in this instance.\u00a0 For those of you who pay attention to this stuff, I cannot just take a brightfield image and divide it out.\u00a0 This is actually a composite (panorama) stitched together from 42 low power shots.\u00a0 The software I use is great (Hugin is the name), but it does interpolation of brightness as part of the stitching process.\u00a0 Thus, the background is not only nonuniform, but very nonlinear.\u00a0 I tried the simple approach of making a gradient from the brightest background to the lightest and dividing that out, but it looked even worse. The ImageJ\/Fij background subtraction didn&#8217;t work all that well, either. I&#8217;m working on some code to do it in an adaptive manner, making local estimations, but I haven&#8217;t finished it yet.\u00a0 If it works, I&#8217;ll post it.\u00a0 If you download it, you can dry different background subtraction methods.<\/p>\n<p>In any case, you can see there&#8217;s a little fibrosis there.\u00a0 Here&#8217;s the polarized Congo Red (once again, click on the image to see noted that everything seems brighter than what he sees in the microscope.\u00a0 He&#8217;s right.\u00a0 It&#8217;s a bit of a long exposure.\u00a0 Looking directly through the microscope, for instance, the background is black.<\/p>\n<p>Here&#8217;s a more random section of myocardium, just to show it&#8217;s not just a cardiac skeleton thing:<\/p>\n<figure id=\"attachment_528\"
